You can only access these forms if you are viewing this website from a work computer.
Online Nomination Form
- HBOS automatically provides all colleagues with free life cover, whether you are a
member of the Scheme or not. This will pay out a sum of four times your basic salary
if you die while still employed by the Group.
With the Nomination Form you can let the Trustee know who you would like to receive
this money. The Trustee will take your wishes into account, but the final decision
rests with the Trustee. Once you have completed the form online click on the 'Submit'
button and it will be automatically sent to Group Pensions.
Online Opt Out Form
- complete this form if you would like to opt out of the Scheme. However, please note that you can
only rejoin the Scheme with the consent of HBOS. Once you have completed the form online click on
the 'Submit' button and it will be automatically sent to Group Pensions.
